What To Expect Before Treatment

Before beginning radiation therapy, you will first meet with a radiation oncologist, who will evaluate your medical history, perform a physical exam, review imaging and lab tests, and discuss whether radiation is an appropriate treatment option.

If you proceed, the next step is simulation, where your body is measured, and positioning and immobilization devices are customized to ensure precise targeting of the radiation. The area to be treated is marked, often using temporary paint or small tattoos, and a specialized CT scan is done for detailed planning. Then, during treatment planning, a team of experts develops a plan to maximize radiation to the tumor while protecting healthy tissue, using advanced software and calculations. Finally, before actual treatment begins, a quality assurance step is performed, including a possible dry run to verify accuracy and safety.

What To Expect During Treatment

During external beam radiation therapy, treatment sessions are painless and noninvasive, similar to getting an X-ray. The radiation is delivered from a machine, often a linear accelerator, and precisely targets the tumor. Treatments are usually done on an outpatient basis, typically five days a week over several weeks. Radiation therapists ensure consistent positioning using any immobilization devices created during simulation. The therapist leaves the room during treatment but monitors you closely and can communicate at any time. The machine may move and make noises during the session, which is normal. The radiation oncologist monitors your progress, adjusting doses or repeating simulation if your body or tumor changes significantly.

Your treatment plan may include a higher dose to a smaller area (a "boost") after initially treating a broader region. In some cases, radiation is combined with chemotherapy, which can increase treatment effectiveness by sensitizing cancer cells. Weekly checkups are conducted to manage side effects and monitor progress. Quality assurance continues throughout treatment through imaging techniques like portal verification to ensure the radiation is accurately targeting the tumor. Regular team meetings ensure your care remains coordinated and responsive to your needs.
